Example of a formulize/pageworks form

Here is an example of a formulize/pageworks form that I'd like to share with the rest of the formulize/pageworks community.

With special thanks to Julian in helping me sort out a couple of tricky issues, particularly on how to provide access to a single-use form for Anonymous Users.

My requirements:
1. Provide a membership application form so that users can fill in the form online, print it and post it in with an attached cheque/check.
2. Enable Anonymous Users to prepare application forms.
3. Make it easy for an existing member to renew their membership by displaying the contents of the user’s last application form so they do not have to re-key their details – they can simply key in any items that may have changed.

FYI Tom, Steve K ran into a similar issue, and after discussing a code change in Formulize, we arrived at a different solution that might work better, at least it's less work than making a Pageworks page. You can read the details in this post:
